The County-District-School (CDS) code provides the California Department of Education (CDE), the Department of Finance (DOF), and postsecondary institutions with a common basis for tracking schools in databases. To ensure that the data remain useful for demographic and fiscal purposes, information stored in the CDS system must be accurate and current.

New CDS Code or Conversion School

General information defining a charter school and information on how to start a charter school may be located at the Charter School General Information [http://www.cde.ca.gov/sp/cs/re/csabout.asp] Web site. For new charter schools, please include a completed CDS code application (Request for a County-District-School (CDS) Code, Form TSD-01) in the charter school application packet. If an existing school is being converted to a charter school, the existing CDS code might remain the same. For charter school conversions, please include a completed "Change Request for a County-District-School (CDS) Code, Form TSD-02" in the charter school application packet.

Changes

Changes to the school name, address, phone number, administrator, enrollment, and grade span can be requested by the district superintendent or authorized charter school representative using the CDS Code Database Change Request Form (TSD-02).

To obtain copies of CDS Code forms, visit the Schools & Districts Web page.
