Your completed form should be sent via e-mail to cahseedatacollections@cde.ca.gov. Please do not change the spreadsheet password. For questions on the CAHSEE waiver and exemption process, contact the California High School Exit Exam (CAHSEE) Office at 916-445-9449, and for questions on the data collection process, contact the Assessment Data Management Office at 916-445-8420.
