LOW VISION REIMBURSEMENT PROGRAM

ANNUAL LETTER OF INTENT*

The _______________________________________(name of SELPA) intends to participate in the Low Vision Reimbursement Program of the California Department of Education. This SELPA serves a total of _________ students who require large print textbooks, video magnifiers (closed circuit televisions), and/or nonprescription optional aids to access the general curriculum. It is anticipated that an approximate total of _____ large print textbooks, _________video magnifiers, and _________nonprescription optional aids will be needed at an estimated cost of $____________ for fiscal year ___________. 

For reimbursement purposes, I certify to the following:

  • All programs for the blind and visually impaired in this SELPA have access to CDE's Braille-N-Teach e-mail list service and will use the site to determine if instructional resources are available on loan from other programs before purchasing new instructional resources.
  • The book titles to be produced in large print have been adopted by the local governing board.
  • CDE’s Price List of Adopted Instructional Materials is searched or publisher contacted to determine the availability of suitable alternate formats before obtaining any material.
  • Queries are made through the CSMT’s Instructional Materials Ordering and Distribution System and the Louis Database of Accessible Media (Outside Source) to determine if the needed material or equipment is available.
  • Other professionals are consulted and assessments are made to determine the best resources for the student.

The SELPA will deliver the state-adopted books (kindergarten through grade 8 only) in large print format that are no longer needed by the students to: California Department of Education, CSMT Media Warehouse, 3740 Seaport Blvd., #20, West Sacramento, CA  95691.

Video magnifiers and nonprescription optical aids that are no longer needed in the SELPA will be shared with adjoining SELPAS or posted on the CDE’s Braille-N-Teach list service as being available at no cost to other programs.

The SELPA will dispose of broken or obsolete equipment in accordance with local district policy in compliance with California Education Code, sections 17545-17555.
