This Management Bulletin provides the United States Department of Agriculture’s (USDA) 2001 revised manual, Accommodating Children with Special Dietary Needs in the School Nutrition Programs (Attachment 1). The 2001 manual includes minor clarifications and information, and supersedes the May 1995 version.
The Nutrition Services Division (NSD) previously disseminated a sample form, Medical Statement to Request Special Meals and/or Accommodations, for sponsors to use in documenting a child’s special dietary needs. NSD revised the form and it is included with this Management Bulletin (Attachment 2). Sponsors must have a completed medical statement on file when a child’s disability, or other accommodation, prevents serving the child a reimbursable meal. NSD also strongly encourages sponsors to use the medical statement when modifications to the texture of food are necessary.
