To reject the grant award, write "DECLINE" on the AO-400, sign and return to the CDE within ten days of receipt of AO-400. We anticipate sending the AO-400s by the end of December 2008. |
Assurances and Certifications (October 2008) Download and keep on file |
Assurances and Certifications are requirements of applicants and grantees as a condition of receiving funds. Applicants do not need to sign and return the general assurances and certification with the application; instead, they must download them and keep them on file to be available for compliance reviews, complaint investigations, or audits. Applicants that participate in the Consolidated Application - ConApp - should already have a copy of the general assurances and certifications on file and do not need to keep a separate copy. Program-specific assurances are not included here and should be listed separately on the Request for Application (RFA). The current general assurances and certifications can be downloaded from our web site. |
End of Period Expenditure Report |
End of Period (EOP) Expenditure Report can be filed with CDE when the EETT Formula funds have been fully expended and/or obligated but no later than September 3, 2010. Upon submission and approval of the EOP, the grant award amount less any reported adjustments will be reimbursed to the LEA. Upon availability, the EOP and filing instructions will be posted on our listserv. Please sign up to join our Lyris Listserv by simply sending a "blank" message to join-eett-formula@mlist.cde.ca.gov. |
